An unannounced inspection was conducted in response to a self-reported incident which occurred on November 12, 2015. Center management report that a three year old child bumped his head on the edge of table while running in the classroom. During the investigation staff were interviewed and a child's record was reviewed. The evidence gathered during the investigation does not support a violation. However, a violation was not reported but found in the area of documentation and will appear on the violation notice. Upon receipt of the inspection documentation, the licensee must develop a plan of correction for each violation. The plan of correction must include the following: ? The steps to correct noncompliance with the standard(s); ? Measures to prevent reoccurrence of noncompliance; ? Person(s) responsible for implementation and monitoring of preventive measure(s); ? Date by which noncompliance will be corrected. The licensee will have ten calendar days from receipt of the inspection documentation to complete the section titled Plan of Correction, sign each page of the documentation and return it to the Licensing Office by 2/28/16. The licensee should retain a copy to be posted at the facility (Supplemental Information is not to be posted due to confidentiality). Results of the inspection documentation are subject to public disclosure and will be posted on the VDSS web site within 15 calendar days, regardless of whether the Plan of Correction section is completed.

Description: Based on record review and interview the center failed to ensure that a written record of a children's serious and minor injuries are maintained and made the day of occurrence. Evidence: 1 - There is no written injury report for an incident which reportedly occurred on November 12, 2015 where a three year old child ran into a table and bumped his head. 2 - Staff 1 and 2 report that the original document was given to the parent and a copy was given to first responders The parent confirmed to staff 1and 2 that she had a copy but could not locate it at this time.

Plan of Correction: As said per social services, we followed procedures as should. Unfortunately, in the chaos, we forgot to retain the accident report. Parents & paramedics confirmed having received a copy. In the future we will make sure we we keep a copy for our records.
