Comments:
An unannounced renewal inspection was conducted on Thursday, February 11, 2016, from approximately 1:30pm until 5:15pm. Present today were: Eleven Infants with three staff; eight toddlers with two staff; Room 2A -nine two year old children with two staff; Room 2B thirteen children with two staff Room 3B - Twenty children with two staff reviewing letter sounds and shapes. Room 4A - ten four year old children with one staff building with magna blocks and creating Valentine bags. Room 4B - ten four year old children with one staff building with magna blocks, legos and coloring. Room 5A (kindergarten and 1st grade ) seventeen children with one staff. Children were observed completing homework assignments, playing computer games, building with magna blocks and legos. Room 5B: (2nd grade)seventeen children with one staff. Children were creating valentine pictures, reading books and completing homework assignments. Seventeen school age children in the clubhouse with one staff completing homework assignments Lunch: Beefy Macaroni Salad, Mandarin Oranges, Blueberry Muffins, Milk PM Snack: Crackers, Cheese Practice Shelter-in-Place Drill: 9/11/15 Practice Evacuation Drill: 1/26/16 Five children's records and four staff records were inspected. The medications for five children were checked.

Violations:
Standard #: 22VAC40-185-210-A-4
Description: Based on inspection of staff records, the center did not ensure all required Lead Teacher qualifications were documented. Evidence: Out of four staff records inspected, Staff #3, record did not contain documentation of the education required for a Lead Teacher.

Plan of Correction: Per Center: Proof of education was requested and placed in staff member's file.

Standard #: 22VAC40-185-270-A
Description: Based on observations, the center did not ensure all areas, inside and outside, were maintained in a safe and operable condition. Evidence: Management and the licensing inspector inspected all playgrounds during today's inspection. Resilient material was both frozen and wet and will be checked during the next inspection. 1.There were roots exposed posing a tripping hazard on the preschool playground. 2.The climbing wooden structure on the toddler playground has peeling paint.

Plan of Correction: Per Center: The items needing to be repaired have been reported to maintenance.

Standard #: 22VAC40-185-510-N
Description: Based on review of children's medications, the center did not ensure all parental authorizations had not expired. Out of five children's medications inspected, medication for Child #1 expired 1/21/16.

Plan of Correction: Per Center: The parent took home the medication until the physician gets the form completed.

Standard #: 22VAC40-185-540-E
Description: Based on inspection of the facility, the center did not ensure all required nonmedical emergency supplies were present in each building.
Evidence:
There was not a battery- operated radio or battery-operated flashlight in the clubhouse.

Plan of Correction: Per Center: A radio and flashlight have been placed in the clubhouse.
