Comments:
An unannounced monitoring inspection was conducted today during a day in which there were no academic events held. There were 37 present with 5 staff and the program has 68 enrolled. The children brought their own lunch from home and lunch bags were labeled and dated. Staff were aware of the children and before heading outside, reminded children on voting activities and to stay with the group. Medication, allergy plans, injury prevention, and drills are all deemed current. Children's files were reviewed as well and deemed current and complete. Please call with questions 703-479-4679

Violations:
Standard #: 22VAC40-185-550-M
Description: Based on record review of 7 injury reports, the center's record of children's minor injuries does not include staff and parent signatures or 2 staff signatures. Evidence: 1. The accident reports reviewed include how, what time, and who contacted the parent but three reports do not include 2 staff signatures when a parent's signature is not obtained.

Plan of Correction: We will get the parent's signature or we will have another staff member sign when we call the parent on the phone.
