It was discussed that careful documentation is necessary when a parent starts and stops the authorizations for the administration of their child's medication(s).
An unannounced mandated monitoring inspection was conducted today. The inspector arrived at 12:05 PM and departed at 2:20 PM. All classrooms were observed. Lunch service, activities and nap time were observed. Medication administration was reviewed. No children's injuries were reported. A sampling of six children's records and four staff records were reviewed. An exit meeting was conducted with the program director, during which the facility's plans of correction for the violations cited were obtained. Standard #: 22VAC40-185-160-C Description: Based upon review of four staff records, the facility has not ensured that they obtain from staff the results of tuberculosis screenings at least every two years. Evidence: 1. The most recent tuberculosis screening provided in the record for staff 3 is dated 1/17/13. 2. Staff 6 was unable to produce a more recent tuberculosis screening for staff 3. Plan of Correction: The facility responded with the following: The TB form will be completed by the end of March.
Standard #: 22VAC40-185-60-A Description: Based upon review of six children's records, the facility has not ensured that the record for each enrolled child contains the written agreement between the parent and the center that the parent will notify the facility within 24 hours if their child or any member of their household contracts a reportable communicable disease. Evidence: 1. The record provided for child 4 did not include the written agreement between the parent and the center that the parent will notify the facility within 24 hours if their child or any member of their household contracts a reportable communicable disease. 2. Staff 5 was unable to provide this signed agreement for child 4. Plan of Correction: The facility responded with the following: The form will be signed within 24 hours by the parent.
Standard #: 22VAC40-185-510-G Description: Based upon review of medication stored for administration to children in care, the facility has not ensured that the center administers only those drugs that were dispensed from a pharmacy and maintained in the original, labeled container. Evidence: There was an albuterol inhaler that was hand labeled for administration to child 7. The albuterol was not in container with the original label as being dispensed by a pharmacy for child 7 under order of the physician (no prescription label). Plan of Correction: The facility responded with the following: Parent was contacted in reference to prescription label and parent will send it with the child to school on 3/4/16.
Standard #: 22VAC40-191-40-D-3-D Description: Based upon review of four staff records, the facility has not ensured that they obtain a search of the central registry before three years since the date of the most recent central registry finding. Evidence: 1. The most recent central registry finding in the record of staff 2 is dated 2/20/2013. 2. The most recent central registry finding in the record of staff 3 is dated 2/12/2013. 3. Staff 6 provided evidence that central registry findings have been requested for staff 2 and staff 3 however they were not requested in a timely fashion to be received before three years since the dated of the most recent central registry findings.
