- Technical Assistance:
Tracking the timing for obtaining children's physicals was discussed with the program director to share with the staff who have this responsibility. Use of volunteers and records needed for volunteers was discussed with the program director.
An unannounced mandated monitoring inspection was conducted today, March 3, 2016. The inspector arrived at 9:20 AM and departed at 11:10 AM. Breakfast service and activities were observed. Medication administration was reviewed. A sampling of five children's records and four staff records were reviewed. An exit meeting was conducted with the program director during which the facility's plans of correction for the cited violations were obtained. Standard #: 22VAC40-185-60-A Description: Based upon review of five children's records and staff interview, the facility has not ensured that each child's record contains documentation of viewing proof of the child's identity and age. Evidence: 1. The records provided for children 1 and 4 do not include documentation that staff have viewed the required proof of the children's identities. 2. Staff 1 was unable to provide documentation of viewing the proof of identity for these two children. Plan of Correction: The facility responded with the following: Proof of identity will be obtained.
Standard #: 22VAC40-185-510-G Description: Based upon review of the medication stored at the facility for administration to children in care, the facility has not ensured that there is written authorization from the parent/guardian and the child's physician (when the medication is to be administered for longer than ten days. Evidence: There was an albuterol inhaler stored for child 1 to be administered as needed. The facility was unable to provide written authorization from the parent and physician for the medication to be administered. Plan of Correction: The facility responded with the following: The parent was contacted during the inspection and the parent and physician authorizations were obtained during the inspection.
