An unannounced complaint investigation regarding supervision ratio and behavioral guidance was conducted from 7:30 a.m-8:30 a.m. with the director. 45 children, ages 4 months to 11 years old, were in the care of six staff. Based on observations, documentation review, and staff interviews, the complaint regarding ratio is determined to be valid. The complaint regarding behavioral guidance is deemed not valid. Description: Based on observation, the center did not maintain the required supervision ratio of staff-to-children. Evidence: At 7:36 a.m. one staff was supervising eleven children in the Preschool 2 classroom. The required ratio of one staff member for every eight children, based on the youngest child in the group, Child A (date of birth: 8/29/13), applies to the entire group.

Plan of Correction: A second staff member immediately entered the classroom. Staff schedules were adjusted.
