The inspector discussed with the administrator standard 22 VAC 40 185 30.A regarding staff training.
An unannounced inspection was conducted on March 9, 2016 from 10:19 a.m. to 2:24 p.m. Upon arrival, there were 21 children ages 5 months to 5 years old with 5 staff present. Records were reviewed for 3 children and 4 staff. There was no medication being administered. Lunch, handwashing and the transition to the rest period were observed in the toddlers? and preschool classes. Areas of noncompliance were found in administration, special care provisions and emergencies and special services. The violations are identified on the violation notice and were discussed with the director during the exit interview. Please complete the columns for "description of action to be taken" and "date to be corrected" for each violation cited on the violation notice. The plans should include steps to correct the noncompliance with the standards; measures to prevent the noncompliance from occurring again; and the person responsible for implementing each step and/or monitoring any preventive measures. Please return the dated, signed copy to licensing office within 10 calendar days of receiving this notice. If you have any questions, please call the licensing inspector.
Standard #: 22VAC40-185-140-A Description: Based on a review of 3 records, the facility did not ensure that each child has a physical examination report on file before the child's attendance or within one month after attendance. Evidence: 1. The record of child #2 did not have documentation of a physical examination report, but the first day of attendance was listed as 11/2/15. 2. Management staff confirmed this. Plan of Correction: 1. Shot record was faxed on 3/9/16 immediately following inspection. Office staff have been advised of not allowing new families to start without physical form on file
Standard #: 22VAC40-185-210-A Description: Based on a review of 4 records, observation and interview, the facility did not ensure that program leaders have fulfilled a high school program completion or the equivalent. Evidence: 1. The record of staff #3 did not have documentation of completion of a high school program or the equivalent. 2. Staff #3 was observed caring for infants as the staff in charge, operating as a program leader. 3. The administrator confirmed this Plan of Correction: 2. Staff members file was copied from main location. We will either get copy from main location and/or have employee bring in a new copy of her diploma by 3/21/16
Standard #: 22VAC40-185-520-C Description: Based on observation, the facility did not ensure that If diaper ointment or cream is used, the product shall be in the original container and labeled with the child's name. :Evidence: The diaper ointment in the infant care room had no child's name on it. When asked, the staff identified the owner. Plan of Correction: 3. Childs ointment was labeled immediately during inspection. Training with staff will be done on 3/25/16 concerning labeling all items that are retrieved from children's bags daily. We will further ensure that all medications are labeled by office personnel before being allowed in the classrooms and that proper documentation has been received.
Standard #: 22VAC40-185-560-G Description: Based on observation, the facility did not ensure that when food is brought from home, the food container shall be sealed and clearly dated and labeled in a way that identifies the owner. Evidence; There were were 2 plastic containers that had powdered substances and no names or date on them. One container had a handwritten label identifying it as cereal. When asked, staff stated that one container had formula and the other probably had oatmeal. Staff stated that the food belonged to child #2. Plan of Correction: Childs food from home was labeled immediately. Parents will be informed via newsletter, messages and face to face of their obligation of labeling every item/ food they bring into the center with child's name, name of item and date.
