- Technical Assistance:
An exit interview was conducted with the director to discuss the results of today's inspection. Consultation was given regarding the emergency preparedness plan.
An unannounced monitoring inspection was conducted today, March 22, 2016 to determine the center's compliance with licensing standards. The inspection was initiated at 3:45pm and concluded at approximately 5:30pm. Upon arrival, the following ratios were observed:
Infant room: 3 infants/ 2 staff
Toddler room: 3 toddlers/ 1 staff
Two-Three year old room: 5 children/ 2 staff
Three-Five year old room: 10 children/2 staff
Children were observed using the restroom, eating snack, playing with toys, playing outdoors, and interacting with staff. Today's snack consisted of a ham sandwich, fresh fruit, and milk. Children's toys, books, and equipment were inspected and found to be age appropriate and in operable condition. All areas used by children were inspected to include the main hallways, children's restrooms, classrooms, gymnasium, and outdoor blacktop play area.
The center had the required postings displayed on the wall upon entering the building. The center's first aid kit and emergency supplies were inspected with the required supplies found. Six children's and two staff records were inspected with the majority of the required information present. The following was also inspected on this day: daily infant reports, annual fire & health inspections, fire/shelter-in-place drills, first aid/CPR/AED certifications, parent agreements, and other pertinent information.

Standard #: 22VAC40-185-160-C Description: Based on the inspection of staff records, the required TB test was not updated. Evidence: During the inspection of two staff records, it was discovered staff #2 had a TB test in the file dated 9/17/13. This TB test expired on 9/17/15; TB test or screenings are required every two years from the date of the initial test or screening. Plan of Correction: Per director: I will have the staff take a TB test or screening within the week. Once the results are received, I will place them in her file.
Standard #: 22VAC40-185-60-A Description: Based on the inspection of children records, the required documentation was not present.
During the inspection of six children records, it was discovered child #1 and #2 did not have the street address listed for the second emergency contact. Licensing standards require two emergency contacts to be listed with the full address and telephone number listed for each contact.
Plan of Correction: Per director: I will have the parent update the information upon pick up.
Standard #: 22VAC40-191-40-D-1-B Description: Based on the inspection of staff records, the required central registry information was incomplete.
During the inspection of two staff records, it was discovered that staff #1 central registry check dated 8/23/14 did not have one of the required boxes checked on the second page. This information is vital to determine the previous and current central registry history of the applicant.
Plan of Correction: Per director: I will call the central registry to check if they can send a completed form and follow through with any recommendations.
