An unannounced monitoring visit was conducted from 2:45-3:45. Thirty eight children were present with four staff on a teacher workday. Children were observed transitioning from the outside playground, hand washing and snack. Snack served today was pretzels and milk. One emergency medication was on-site today. Reviewed emergency drills, staff trainings, ratios and supervision, nutrition and physical plant. Center Director was on sick leave and a Head Teacher from another program was present. Rooms are well maintained and supplied with age appropriate materials and equipment for school-age children. Room reflects center year long Global Adventures programming theme. Standard #: 22VAC40-185-240-D-4 Description: Based on staff and child statement, a child with an emergency medication on-site was not in the care of a staff member with Medication Administration Training (MAT) certification. Evidence: Child #1 arrived approximately 7:20 and was at the center with emergency medication on-site. A staff member with MAT certification did not arrive until 9:00. Plan of Correction: Staffing patterns will be adjusted to ensure a staff has MAT certification at all times the center is open.
Standard #: 22VAC40-185-510-A Description: Based on record review, a medication authorization did not contain a parent signature. Evidence: An authorization for a long term medication contained the physician signature but was missing the parent written signature. Plan of Correction: Center staff will have the parent sign the authorization form today.
Standard #: 22VAC40-185-550-M Description: Based on record review, an injury report was missing information. Evidence: The report did not include two signatures as required (either parent and staff or two staff members). Plan of Correction: Will complete with two signatures.
