22VAC40-185 STAFFING AND SUPERVISION.
Today a self-reported other visit was conducted with the Principal regarding concerns about a toddler being fed another toddler's bottle. After interviewing staff violations were cited. Please see violation notice. If you have any questions, please e-mail me at Mayasah.Thomas@dss.virginia.gov
Standard #: 22VAC40-185-340-A Description: Based on interviews, staff did not ensure care, protection and guidance while supervising toddlers during feeding. Evidence: On March 16, 2016 ,at approximately 12:00noon Child A was given Child B's breast milk bottle. Child B's bottle was labeled with the child's full name and date however Staff #1 failed to read the label to ensure that the right bottle was given to the correct child. At around 3:30-4:00pm while preparing for snack and cleaning bottles, Staff #1 discovered that Child A was given the wrong bottle. Plan of Correction: Principal stated that immediately parents were notified and they continue to monitor to ensure that the child is doing okay. Principal also stated that staff was retrained on 3/17 on bottle verification and modified the daily information sheet to be able to document more information regarding feeding for parents and teachers.
