22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted from 2:30 p.m.-5:00 p.m. with the center director and assistant director. 75 children, ranging in age from four months through 11 years, were in the care of 14 staff. Children were observed eating afternoon snack, listening to an ABC book, having tummy time, and playing outside on the playground. Afternoon snack today was pretzel wheels, cheese slices, and water. The last emergency escape drill was 2/10/16. Both the fire and health inspections are current. Standard #: 22VAC40-185-160-A Description: Based on review of two staff records, the center did not have documentation of a tuberculosis test or screening for a staff within 21 days of hire or within 12 months prior to hire. Evidence: Staff A's (hired 1/26/15) tuberculosis test reading was dated 3/4/16. Plan of Correction: With future new staff, we will assure that their tb tests are less than 12 months old or done promptly within 21 days or less after employment.
Standard #: 22VAC40-185-70-A Description: Based on a review of two staff records, the center did not maintain complete staff records. Evidence: Staff A's (hired 1/26/16) records did not include documentation of two or more reference checks. Plan of Correction: Two references will be called tonight.
Standard #: 22VAC40-185-80-A Description: Based on review of written classroom attendance records and staff interview, the center did not maintain an accurate written record of children in attendance. Evidence: On 3/30/2016, there were eleven children marked as present on the written attendance record in the infant classroom. However, there were eight children physically present in the classroom. Plan of Correction: The written attendance record was fixed and addressed during the inspection. Maintaining accurate attendance will be addressed with all staff at the April staff meeting.
