Phase II training is being offered on April 5, 2016 at the Abingdon office.
An unannounced monitoring inspection was conducted today, 3/22/16. Upon arrival to the center a volunteer was getting ready for the arrival of the children. The children began arriving at 3:00 pm. A review of the previous violations was also conducted during today's inspection. A violation notice was completed regarding the repeat violations and the violations found during today's inspection. Snacks, hand washing and table sanitizing was also observed. Following snack the children that wanted went swimming. The remaining children worked on homework, played with the available toys/games or went outside. A review of four children's records was conducted. There are 40 children present today with four staff and one volunteer. One new staff was present and staff records were reviewed on 3/24/16 at the main office. An acknowledgment of Inspection Conducted & Licensee Responsibility was reviewed, signed and left at the center. Should you have any questions please do not hesitate to call. Standard #: 22VAC40-185-130-A Description: Based on review of the children's records the center failed to obtain an immunization for a child in care. Evidence: 1. Child #2 began care 8/10/15 and there is no documentation of immunizations on file. One of four records reviewed. Plan of Correction: The center will get these records from the parent as soon as possible.
Standard #: 22VAC40-185-140-A Description: Based on review of children's records the center failed to obtain a physical for a child in care. Evidence: 1. Child # 2 began care 8/10/15 and there is no documentation of a physical on file. One of four records reviewed. Plan of Correction: The center will get these records from the parent as soon as possible.
Standard #: 22VAC40-185-70-A Description: Based on observation the center failed to maintain staff information at the center. Evidence: 1. The center staff did nt have emergency information on site at the center. Plan of Correction: The center will have all the emergency contacts at the center.
Standard #: 22VAC40-185-260-B Description: Based on review of available reports the center failed to obtain an annual health inspection. Evidence: 1. The health inspection documentation available during today's inspection is dated 3/25/13. Plan of Correction: The staff will obtain a copy of the health inspection from the Coomes center.
Standard #: 22VAC40-185-550-D Description: Based on review of the monthly evacuation drills and shelter-in-place drills the center failed to conduct these as required. Evidence: 1. The last documented fire drill that was available during this inspection was dated 8/26/14. These drills are to be conducted monthly. 2. The last documented shelter-in-place drill that was available during today's inspection was dated 8/26/14. These are to be conducted twice annually. Plan of Correction: The center staff will begin practicing the fire drills and shelter in place drills and will document these drills and will be kept a log sheet in a folder.
Standard #: 22VAC40-80-120-E-2 Description: Based on observation during the inspection the center failed to post the violation notice. Evidence: 1. The violation notice from 10/22/15 was not posted. The PD could not locate the violation notice. Plan of Correction: The current violation notice will be posted in an area accessible to the public.
