22VAC40-191 Background Checks (22VAC40-191)
A Renewal inspection was conducted in the Center on March 30, 2016. The inspection was unannounced. The LI observed 13 children present with 2 staff. One additional administrative staff was also present in the Center. The children were observed during outdoor play on the fenced playground and the children were served a snack. The children were also observed in their classroom as they participated in varied activities which included: story time, music, hand washing, nap time and dismissal at the end of the program day. The children were also observed during the lunch meal in the school cafeteria. A sample of 5 of 18 children's records were reviewed. Documentation of the Center's annual fire and health inspections was reviewed along with documentation of the practiced emergency evacuation and shelter-in-place drills. A child's prescribed medication was observed by the LI, written documentation for long term administration was in place. Three (3) partial staff records were reviewed. The inspection was conducted from approximately 10:50am to 3:15pm. The findings were reviewed with staff at the exit. Complete staff records are maintained off site and were reviewed on March 31, 2016. A discussion was held with Administrative staff regarding the Center's policies for meals/snacks and the scheduled nap/rest time for the children. Standard #: 22VAC40-185-70-A Description: Based on review of staff records on site, the Center failed to maintain the name, address, and telephone number of an emergency contact person for each staff member working at the Center. Evidence: Staff 1 who was recently employed in the Center (02/2016) did not have information available for review to identify an emergency contact. Upon notice, the staff member obtained the form, completed the information and placed in the record. Plan of Correction: The Center staff will ensure all information as required for on site record retention is maintained in each staff's record.
Standard #: 22VAC40-185-540-E Description: Based on observations, the Center failed to ensure a working, battery operated flashlight was available. Evidence: The LI and a staff member tested the flashlight and it was not in working order. The extra batteries were replaced in the flashlight by the staff and the flashlight did not work. Plan of Correction: Staff will replace the flashlight. Staff will check on going to ensure the flashlight is in working order.
Standard #: 22VAC40-185-560-F Description: Based on observations by the LI and staff interview, the Center failed to follow the most recent, age-appropriate nutritional requirements of a recognized authority such as USDA. Evidence: 1. The LI observed the children during the morning snack. The children were served pretzels and water. The snack did not consist of two of the four food groups. The snack menu posted in the center did not indicate that at least two of the four food groups were served to the children each day. There were multiple days on the menu that listed only 1 food group. 2. The Center failed to provide second helpings of food listed in the USDA's child and adult care meal patterns . The LI observed the children during the lunch meal in the school cafeteria. Two children requested second helpings from staff and were denied. The LI inquired with staff regarding second helpings for children. The staff reported to the LI that the contract with the school and reimbursement only allows for 1 plate serving per child and that the program would be charged for additional servings. Plan of Correction: Administrative staff will review the center's food policies (meals and snacks) with all Center staff to ensure children are provided second portions upon request. All snacks served will consist of two components from the four food groups.
