Technical Assistance:
Written information was left regarding MAT training requirements and child abuse reporting.

Comments:
An unannounced monitoring inspection was conducted on 3/31/2016 from 10:05am - 11:35am. The following staff to child ratios were reviewed: 2's & 3's - 12 x 2 staff 4's - 4 x 1 staff SA - 5 x 1 staff Children were observed during morning program time, morning snack and during outdoor play. Records were reviewed for four children and two staff. The inspector additionally reviewed: emergency supplies, posted evacuation plans, documentation of emergency practice drills, documentation of injuries, hand washing, diapering, and topical ointments. *No medication is being administered at this time.

Standard #: 22VAC40-185-280-G
Description: Based on observation, the center failed to ensure that if hazardous substances are not kept in original containers, the substitute containers shall clearly indicate their contents and shall not resemble food or beverage containers. Evidence: A spray bottle of bleach and water solution was not labeled to indicate the contents of the bottle.

Plan of Correction: The bottle will be labeled to indicate the contents today.

Standard #: 22VAC40-185-560-K
Description: Based on observation, the center failed to ensure that tables shall be sanitized after snacks. *"Sanitized" means treated in such a way to remove bacteria and viruses from inanimate surfaces through using a disinfectant solution. The surface of item is sprayed with the disinfectant solution and allowed to air dry after use of the disinfectant solution. Evidence: After completion of afternoon snack the tables in the four year old class were sanitized with bleach and water. However, staff used a cloth towel to immediately wipe the tables instead of allowing them to air dry as required.

Plan of Correction: Staff will be retrained on the proper procedures for sanitizing tables after meals and snacks.
