Comments:
An unannounced monitoring inspection was conducted on 3/30/2016 from 12:35pm - 3:10pm. The following staff to child ratios were observed: *Infants - 5 x 2 staff *Toddlers - 7 x 2 staff *2's - 16 x 2 staff *3's & 4's - 12 x 2 staff *School Age - 18 x 1 staff Children were observed during nap and afternoon snack. The school age class was observed engaged in quite activities which included games, reading and art activities. Diapering was reviewed in the infant and toddler class. Hand washing was observed in all classrooms. The inspector additionally reviewed records for four children, three staff, emergency supplies, injury documentation, posted evacuation procedures, the posted menu, topical ointments, infant standards, the outdoor play area, staff training and documentation of emergency practice drills. *No medication is being administered at this time.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to ensure that outside equipment of the center shall be maintained a safe condition. Evidence: A broken plastic storage container was observed on sidewalk of the school age playground. The broken area of plastic had three sharp points.

Plan of Correction: This container was removed during the inspection.
