22VAC40-191 Background Checks (22VAC40-191)

An unannounced monitoring inspection was conducted on April 12, 2016. Upon arrival to the facility there were 12 children present with two staff members. Parent volunteers were present as they were participating in Parent Involvement week and the Week of the Young Child. The children were observed having family style breakfast, playing in centers and during circle time. A special event was also observed for Taco Tuesday, where the children were building their own tacos. The outdoor area was not inspected due to inclement weather. One staff record was reviewed. Three children's records were reviewed and one medication was reviewed. Exit interview was conducted with the Program director.

Standard #: 22VAC40-185-40-H
Description: Based on observation and review of documentation, the licensee did not maintain public liability insurance for bodily injury for each center site with a minimum limit of at least $500,000 each occurrence and with a minimum limit of $500,000 aggregate. Evidence: During the inspection conducted on April 12, 2016, the liability insurance documentation provided by the facility expired on 2/8/16.

Plan of Correction: I will ask the Human Resources Manager to update our paperwork.

Standard #: 22VAC40-185-60-A
Description: Based on record review, in one of three children's records reviewed, the licensee did not ensure that each child's record was documented with all required information. Evidence: During the inspection conducted on April 12, 2016, in the record for Child #2 there was no documentation of previous child day care and schools attended by the child and the name of any additional programs or schools that the child is concurrently attending and the grade or class level.

Plan of Correction: I will ask the parent to update the record.

Standard #: 22VAC40-185-550-D
Description: Based on review, the licensee did not ensure that a minimum of two shelter-in-place practice drills were conducted per year for the most likely to occur scenarios. Evidence: During the inspection conducted on April 12, 2016, there was documentation of one of two required shelter-in-place drills practiced in 2015.

Plan of Correction: I will make sure to do two a year.

Standard #: 22VAC40-191-40-D-3-D
Description: Based on record review, the licensee did not ensure that staff provide a search of the central registry before three years since the date of the most recent central registry finding. Evidence: During the inspection conducted on April 12, 2016, the most recent central registry finding for Staff #1 expired and was dated 11/13/12.

Plan of Correction: We have sent off for the background check. The results are pending.
