Description: Based on records reviewed, it was determined that 3 of 5 staff records did not include documentation of the education required for the Lead Teacher positions they were hired/working. Evidence: 1. Staff #3 - Began working as a Lead Teacher in January 2014. 2. Staff #4 - Began working as a Lead Teacher in February 2016. 3. Staff #5 - Began working as a Lead Teacher in February 2016.

Plan of Correction: The director collected required documentation from all noted staff.

Standard #: 22VAC40-185-280-B
Description: Based on observations made, it was determined that hazardous substances were not kept in a locked place. Evidence: A spray air freshener and 2 body spray products were found on a counter top in a 2 year-old classroom.

Plan of Correction: The director will review with staff proper lock-up procedures. Items were immediately locked up.

Standard #: 22VAC40-185-350-E-3
Description: Based on observations made, it was determined that a staff-to-child ratio of 1:8 was maintained in the 2 year-old classroom. Evidence: The ratio was 1 staff to 13 children, for one minute or less, when a staff person left the room to rinse a potty chair.

Plan of Correction: The director will review proper ratios with staff.

Standard #: 22VAC40-185-500-A
Description: Based on observations made, it was determined that a child's hands were not washed after being diapered in the infant room.

Plan of Correction: The director reviewed diapering procedures with all staff

Standard #: 22VAC40-185-510-J
Description: Based on observations made, it was determined that medications were not kept in a locked place. Evidence: Medications were found to be in a unlocked cabinet in the office.

Plan of Correction: The director has locked all medications in a lock box in the office

Standard #: 22VAC40-80-120-E-2
Description: Based on observations made, it was determined that the most recent inspection findings were not posted in a manner that was readily viewable by the public. Evidence: The report was on a counter top behind the front desk. One would need to walk behind the desk to read the report.

Plan of Correction: The most recent inspection findings are now posted in the main part of the lobby
