- Technical Assistance:
1. Written injury reports should be reviewed for accuracy on an ongoing basis. 2. Personal bags should not be placed on cots. 3. Standing water on the playground should be dumped promptly, and not permitted to sit for days.
Conducted an unannounced monitoring inspection 11:25am-1:30pm. Observed 44 children + 10 direct-care staff + 2 administrative staff. Ratios and supervision were in compliance. Children were observed having story time, creative play, sensory table activities, playing with blocks and puzzles, having a soccer lesson, washing hands, and eating lunch. The infants were having play time on the floor, being held and fed, and taking naps. Staff were actively involved with the children and assisting them with tasks in all groups. Lunch served: beef, vegetarian quinoa, farrow, peas, banana, and milk. The center was found to be clean and sufficiently supplied with toys and equipment for the children. Medication management was in compliance. Required documents were posted. Record keeping violations found. Inspection completed 11-25am-1:30pm. Standard #: 22VAC40-185-510-C Description: Based on records reviewed, it was determined that documentation for 2 of 3 long-term medications was not in compliance with center procedures, and did not include the physician's authorization. Evidence: Child #1 - One of the medications had a written authorization signed by the parent on 9/1/15, but no physician authorization; and the other topical medicated gel had a written authorization signed by the parent on 10/1/15, but no physician authorization.
