During the investigation we discussed emergency evacuations, first aid kits, documentation of staff presence in classrooms, and documentation of children's attendance.
A complaint was received on February 11, 2016 and initiated on the same day. Allegations of lack of following ratios in the late afternoon, incomplete first aid kits and lack of emergency evacuation drills. During the investigation on site inspections and collection of documentation was completed on 3/3/16, 3/18/16 and 4/4/16. The investigation was concluded on 4/4/16. Classroom observations were conducted, children's attendance records and staff time sheets were reviewed. Interviews were conducted with staff. The information gathered does not support the allegations lack of ratios and incomplete first aid kit. Therefore, that portion of the complaint is not valid. However, the information gathered during the investigation does support the allegation of lack of documentation of emergency evacuation drills. Therefore, the complaint is valid. A violation regarding emergency evacuation drills appears on the violation notice. Upon receipt of the inspection documentation, the licensee must develop a plan of correction for each violation. The plan of correction must include the following: ? The steps to correct noncompliance with the standard(s); ? Measures to prevent reoccurrence of noncompliance; ? Person(s) responsible for implementation and monitoring of preventive measure(s); ? Date by which noncompliance will be corrected. The licensee will have ten calendar days from receipt of the inspection documentation to complete the section titled Plan of Correction, sign each page of the documentation and return it to the Licensing Office by 4/22/16. The licensee should retain a copy to be posted at the facility (Supplemental Information is not to be posted due to confidentiality). Results of the inspection documentation are subject to public disclosure and will be posted on the VDSS web site within 15 calendar days, regardless of whether the Plan of Correction section is completed.
Standard #: 22VAC40-185-550-E Complaint related: Yes Description: Based on observation and interview the center failed to maintain a record of the dates of the practice drills for one year. Documentation provided during the inspection is dated August 2014 - July 2015 and July 2015 -December 2015. 1 - The records reviewed does not document two shelter-in-place drills from August 2014 through August 2015. 2 - Staff 1 confirmed that the documentation was not present in the center records. Staff 1 stated that she was not the center director during the 2014 - 2015 time frame and therefore, could not account for the lack of information regarding the drills. Plan of Correction: emergency log will be up to date and show that we have completed and documented the proper drills in our monthly log. This was shown to be up-to-date November through march of 2016.
