Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records, it was determined that one record was not complete. Evidence: Child record #5 does not include the mother's work contact information.

Plan of Correction: I will ask the parent to update the information in the file.

Standard #: 22VAC40-185-80-A
Description: Based on a review of administrative documents, it was determined that one classroom attendance was not correct. Evidence: The classroom attendance in Room 7 indicated a total of 6 children in care, however, a total of 8 children were present and in care.

Plan of Correction: Staff will be reminded about the importance of making sure the attendance is always correct.

Standard #: 22VAC40-185-500-A
Description: Based on observations, it was determined that the children in one classroom did not wash their hands before snack as required. Evidence: In Room 6, after washing their hands the children proceeded to sit on the carpet and read books. After putting their books away, the children got out their snack and proceeded to sit down to eat. This process does not ensure that the children wash their hands immediately prior to eating.

Plan of Correction: All classes will update their routine to make sure the children wash their hands immediately before eating and without engaging in any activities between hand washing and eating.
