Comments:
An unannounced monitoring inspection was conducted on 4/19/2016 from 10:40am - 12:50pm. The following staff to child ratios were observed: *2's - 10 x 2 staff *3's - 9 x 2 staff *4's - 17 x 2 staff Children were observed during lunch and nap. Hand washing was observed. Additionally reviewed: diapering procedures, medication, emergency supplies, documentation of injuries, four children's records, two staff records, posted evacuation plans, documentation of emergency practice drills, and transportation procedures. All violations from the 11/17/2015 inspection have been corrected.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center failed to ensure that hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. If a key is used, the key shall not be accessible to the children. Evidence: 1. A can of aerosol spray air freshener was observed stored on a high shelf in the unlocked closet of the four-year old classroom. 2. The label on this product indicated the contacts as hazardous.

Plan of Correction: This product was locked up during the inspection. Additionally, staff were reminded to lock up all chemicals.
