Technical Assistance:

The licensing inspector conducted an unannounced monitoring inspection April 21, 2016 from approximately 1:15pm to 4:00pm. The following ratios were observed with a total of 30 children present during the inspection:
Toddlers - 5 children with 1 staff
Twos - 7 children with 1 staff
Threes (Preschool) ? 9 children with 1 staff
Fours & Fives (Pre-K) - 9 children with 1 staff

When the licensing inspector arrived at the facility children were observed sleeping at naptime and being supervised by the staff. Later, after the children woke from naptime, children were observed eating snack. Snack consisted of goldfish and juice. After the children finished snack children were observed playing outside where they blew bubbles, played on climbing equipment, playing in the sand box and riding tricycles. Children were also observed learning about recycling and creating sticker collages, and taking bathroom breaks. Staff was observed interacting with the children and providing assistance when needed. All areas of the center including classrooms, hallways, bathrooms, kitchen, and playgrounds were inspected. The facility was equipped with toys and supplies and items were available to the children.

The following information was reviewed: emergency supplies, injury reports, fire and shelter-in-place drills, daily attendance, parent agreements, and staff training information. The owner stated medication is not administered at this time.

Standard #: 22VAC40-185-60-A
Description: Based on review of four children's records, one record did not contain the required information. Evidence: During the inspection, the licensing inspector observed the record for Child #4 (enrollment date 01/04/16) did not contain information regarding current school child is attending. The owner stated they began using a different form and mistakenly omitted the question regarding current school arrangements.

Plan of Correction: Per owner - we are using the Virginia Department of Social Services forms that have the required information. I requested the information from the family.

Standard #: 22VAC40-185-260-B
Description: Based on documentation review, the facility did not obtain a health inspection from the local health department at least on an annual basis. Evidence: During the inspection, the licensing inspector reviewed the most current health inspection which was dated 11/20/2014. Approval from the local health department is required at least on an annual basis.

Plan of Correction: Per owner - We did not realize we needed to request an inspection from the local health department. We have contacted the local health department and an inspection is scheduled for 04/27/2016.
