Discussed with the center director hand washing procedures during diapering.
An unannounced renewal inspection was conducted today from 9:30am-11:15am. There were 46 children (infants-5yrs.) directly supervised by 13 staff. The physical plant, 4 staff records, 4 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in circle time, singing songs and playing outside. Diapering procedures were also observed. Staff-to-child ratios were in compliance. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-270-A Description: Based on direct observation, areas of the center were not maintained in a clean, safe and operable condition. Evidence: Peeling paint was observed along the wall of the 2's bathroom and also on the wall at the top of the stairwell, entering into the preschool classroom. Plan of Correction: A painter was hired to take care of all the issues. Teachers were reminded not to glue posters or any other work on the wall for display. This causes paint to stick to posters; painters tape will be used to avoid damaging the paint. The work started on 4/17/16 and will be finished by 4/23/16.
Standard #: 22VAC40-185-280-B Description: Based on direct observation, hazardous substances were not kept in a locked place that prevents access by children. Evidence: A cabinet containing cleaning products was observed unlocked in the Preschool bathroom and in the Toddler classroom. A locking device was observed on the cabinet doors but was not secured in a locked manner. Plan of Correction: Cabinets were locked immediately on 4/15/16; teachers are reminded that these should be locked at all times while not being used even if the children cannot reach them.
Standard #: 22VAC40-185-510-C Description: Based on medication review, the center did not follow their medication policies and procedures. Evidence: 1. Documentation of a written authorization from the child's physician and parent was not observed for Child 5, who had a long term medication on site. 2. Documentation of a written authorization from the child's parent was not observed for Child 7, who had an over-the-counter medication on site. An over-the-counter skin products form, which is an incorrect form, was observed for Child 7. Plan of Correction: Parents of child 5 were notified for the need for authorization from the Doctor on 4/15/16. The medication for child 7 was returned to the parents because it is not used anymore.
Standard #: 22VAC40-185-510-G Description: Based on medication review, a child's medication was not maintained in the original, labeled container. Evidence: An emergency medication observed for Child 7 was kept in a ziploc bag and the medication prescription label was not attached. Plan of Correction: Parents were notified the morning of 4/15/16 and we received the required documentation the afternoon of 4/15/16.
Standard #: 22VAC40-185-550-M Description: Based on record review, written records of children's injuries were incomplete. Evidence: 1. The date that the parent was notified of the injury was not observed on three of five injury records reviewed. 2. The time that the parent was notified of the injury was not observed on four of five injury records reviewed. 3. The future action to prevent the recurrence of the injury was not observed on one of five injury records reviewed. Plan of Correction: We generated a new form on 4/15/16 which includes the date, time, and how the parents were notified (by phone, e-mail, or in person)
