An unannounced monitoring inspection was conducted from 10:00 A.M. until 12:00 P.M. At the time of the entrance, 19 children were in care with three staff members present. The sample size consisted of three children's records and one child's medication and supporting documents. Children and staff were playing in the gym at the beginning of the inspection since it was raining outside. Children and staff were observed during centers, clean-up, educational activities in small groups, art activities, hand washing, and lunch. The staff were observed during transitions and behavioral management. Areas of non-compliance are identified on the violation notice. Standard #: 22VAC40-185-550-M Description: Based on record review, two out of three injury reports reviewed did not contain staff and parent signatures or two staff signatures. Evidence: Injury reports in the records of child 2 dated 12/14/15 and child 3 dated 1/15/16 only contained one signature on each. Plan of Correction: The staff will ensure that injury reports contain staff and parent signatures or two staff signatures in the future.
