An unannounced monitoring inspection was conducted from 3:55 p.m. to 5:15 p.m. At the time of entrance the following adult to child ratios were observed: 23 children, ages 4 to 9 years old, with 3 staff. The records sample size consisted of 3 children?s records. Staff records are housed off site and will be reviewed at a separate inspection. Children were observed completing homework, playing with table games, puzzles, construction materials, eating dinner (cheese pizza, tossed salad, apple sauce and milk) and having an afternoon snack (cinnamon roll and orange juice). Medication administration was not observed. Staff state that there are no children being administered medication while in care at this time. Standard #: 22VAC40-185-260-A Description: Based on a review of available documentation, the licensee was unable to provide to the licensing inspector a current annual fire report from the fire official having jurisdiction. Evidence: The fire inspection posted on the information board for the before and after school program was dated 12/10/14 and was conducted more than a year ago. When asked for a more recent inspection report, staff were not able to provide one. Staff stated that they depend on the school to supply these reports and that they had not received a more recent fire inspection report to date. Plan of Correction: Staff stated that they would contact the school office personnel in the morning to obtain a copy of the most recent fire inspection report. In addition, they will request a new report annually as the old report nears expiration at 1 year.
Standard #: 22VAC40-185-280-B Description: Based on observation, the licensee did not ensure that hazardous substances are kept in a locked place using a safe locking method that prevents access by children. Evidence: The licensing inspector observed a pump bottle of hand sanitizer sitting on a microwave cart in the back of the cafeteria where children are cared for. The hand sanitizer was labeled with several warnings to include "keep out of reach of children", "contact a physician if ingested" and "Warning: Flammable". Staff stated that the hand sanitizer was left by the public school staff from the current school day and that children are carefully monitored as they pass that area of the cafeteria and do not have activities or play in that end of the room. Children were only observed in the vicinity of the hand sanitizer when they were escorted by a staff person to the restroom. However, the hand sanitizer was located out in the open in the same room with the children and was easily accessible. Plan of Correction: Staff removed the hand sanitizer from the cart and locked it in a cabinet during the inspection. Staff staff stated that they will scan the area for hazards each session at the time of arrival and will lock up items in their cabinet, as needed, while children are in care, regardless of who the items belong to.
