Reviewed asbestos requirements and center management plan with Center Director.
An unannounced monitoring visit was conducted from 10:15-11:30. Three classrooms were in attendance with ratios maintained by direct care staff. Children were observed participating in circle time, free play on the playground, transition to handwashing, singing and preparation for lunch. Lunch served today was turkey sloppy joe sandwich, veggies and milk. Emergency drills and trainings were up to date. Areas of non-compliance are identified in this report. Standard #: 22VAC40-185-260-C Description: Based on documentation and center management statement, the center did not follow the current asbestos management plan and do a visually check every six months. Evidence: Last documented walk through of areas containing documented asbestos was conducted April 2015. The new Center Director did not do a walkthrough in October 2015. Plan of Correction: On April 18, 2016 the Director was trained by a licensed management planner. An inspection (visual check) was performed on this date.
Standard #: 22VAC40-185-270-A Description: Based on observation, an area of the center is not being maintained in a safe manner. Evidence: The porch landing leading into the center has one slate piece that is eroding and the slate is disintegrating causing a potential tripping hazard. Plan of Correction: The slate will be replaced by May 6, 2016. It has been temporarily covered by an outdoor mat.
Standard #: 22VAC40-185-550-M Description: Based on record review, injury forms are incomplete. Evidence: 15 injury reports were reviewed during the inspection. 5 were missing "Future action to prevent reoccurrence" and 4 were missing the time parents were notified. Plan of Correction: Staff will be retrained on completing injury forms at the next staff meeting.
Standard #: 22VAC40-191-40-D-3-D Description: Based on record review, 1 of 10 staff members had an expired Sworn Disclosure Statement (SDS) Evidence: Staff #1's SDS was older than three years as it had not been renewed prior to 3/13/2016. Plan of Correction: Staff #1's SDS was updated immediately following the inspection on 4/6/2016.
