22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted today from 10:08am-11:55pm. There were 60 (infants-5yrs.) directly supervised by 13 staff. The physical plant, 3 staff records, 3 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, circle time, individual activities, and preparing for lunch. There was an abundant supply of books, toys, and materials for the children. The center was clean and organized. Areas of non-compliance are identified in this report. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. Standard #: 22VAC40-185-130-A Description: Based on review, the facility failed to ensure that the center obtains documentation of immunizations before the child can attend. Evidence: Reviewed 3 children's records and found that CH1 did not have documentation of an immunization record. CH1 has been in care since 3/28/16. Plan of Correction: Parent was emailed following licensing visit and provided copy of immunization record same day - 4/14/16 (sic)
Standard #: 22VAC40-185-270-A Description: Based on observation, the facility failed to ensure that the areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. Evidence: Observed peeling paint in the Older Infants Room, Toddler Room and Computer Room. Plan of Correction: Paint was corrected in older Infants, Toddler and computer classroom (sic)
Standard #: 22VAC40-185-280-B Description: Based on observation, the facility failed to ensure that hazardous substances must be kept in a locked place using a safe locking method that prevents access by children. Evidence: Observed bleach water spray bottle on a shelf accessible to children. Plan of Correction: Bleach bottles will be locked away in cabinet when not in use. (sic)
Standard #: 22VAC40-185-280-H Description: Based on observation, the facility failed to ensure that cosmetics, medications, or other harmful agents shall not be stored in areas, purses or pockets that are accessible to children. Evidence: 1) Observed that there was a staff bag with medications on the floor of the Preschool classroom. 2) Observed 2 staff bags accessible to children in the Junior K-Prep Room. Plan of Correction: All staff have been communicated to keep/ensure all bags are locked away in a cabinet during operating hours. (sic)
Standard #: 22VAC40-185-280-I Description: Based on observation, the facility failed to ensure the hazardous art and craft materials shall not be used with children. Evidence: Observed that there was a container of glitter in an unlocked cabinet in the Preschool Room, accessible to children ages 2 1/2 to 3 1/2 years of age. Reviewed on the label of the glitter that it was "not intended for children under 3 years of age". Plan of Correction: Glitter was removed following licensing visit and new locked has been placed on cabinet. (sic)
Standard #: 22VAC40-185-290-3 Description: Based on observation, the facility failed to ensure that all electrical outlets have protective covers. Evidence: Observed in the hallway between the Computer Room and the Jr K-Prep Room an uncovered electrical outlet. Plan of Correction: Protective cover was placed in electrical outlet during visit. (sic)
