Comments:
An unannounced monitoring inspection was conducted on 5/3/16 from 3:10pm - 4:20pm. At the time of entrance there were 47 school age children in care with nine staff. Children were observed participating in completing homework, working in the computer lab, and eating snack. Records were reviewed for five children and five staff. Medication, emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and returned it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Violations:
Standard #: 22VAC40-191-60-C-1
Description: Based on a review of five staff records, it was determined that the facility did not deny continued employment of a staff who did not have a criminal history record report within 30 days of employment. Evidence: 1. The record for staff #1 (date of hire 1/19/16) contain documentation of a completed criminal history record report that was dated 4/18/16. 2. The record for staff #2 (date of hire 1/25/16) contain documentation of a completed criminal history record report that was dated 4/15/16. 3. The record for staff #3 (date of hire 3/3/16) contain documentation of a completed criminal history record report that was dated 4/18/16. 4. The record for staff #4 (date of hire 3/15/16) contain documentation of a completed criminal history record report that was dated 4/18/16.

Plan of Correction: The Program Director will call the State Police to check on future criminal history checks if not received back within 30 days. The Program Director will also expedite the process of getting approval for the checks so that they will be sent in a more timely manner.
