- Technical Assistance:
An unannounced monitoring inspection was conducted on 5/9/16 at 10:15 a. m. and concluded at 12:45 p. m. The inspector met with the program director and some but not all Child Day Center standards were reviewed. The following staff-to-child ratios and activities were observed: Infant A (6wk-8m) Six infants with two staff. The infants were having free play. Infant B (8m-16m) Eight infants with two staff. The teachers were observed taking the infants outside on a buggy ride. Toddler (16m-24m) Eight children with two staff. The children were observed painting, and having free play. Two Years There were eight children with one staff. The children and staff were observed playing outside and later came inside to their classroom. Preschool IA (2 1/2 years-3 years) There were 14 children with two staff. The children were having free play in home living, listening to music, and having stories on the carpet. Preschool 2A (Young 3 years) There were 20 children with two staff. The children were sitting at the table with table toys. Later, the children and staff were observed outside on playground. Preschool 2B (3-4 years) There were 20 children with two staff. The children were sitting down and having story time. Pre-K (4-5 years) There were 10 children with one staff. The children were having a dance class. Lunch consisted of: Macaroni and Cheese with Beef crumbles Broccoli Peaches Milk Three staff records were reviewed. Two children records reviewed. Two children medications reviewed. Several staff are trained in MAT (medication administration trained). Conversations were held with several children and staff. Two classroom observations conducted. All classrooms are nicely equipped with a variety of equipment, toys and supplies. The most recent evacuation drill was documented on 4/27/16. The most recent shelter-in-place drill was documented on 2/24/16.
Standard #: 22VAC40-185-510-C Description: Based on review of medication procedures the center did not return an expired medication to the parent. Evidence: On 5/9/16, after reviewing two children's medications, child #1 had a medication that had an expiration date of 1/2016. Plan of Correction: I have sent parent an email requesting an updated medication and will return the expired medication to parent today.
Standard #: 22VAC40-185-540-E Description: Based on observation and discussion with staff the center did not have all required emergency supplies. Evidence: On 5/9/16, the battery-operated radio was not operable. Plan of Correction: I will purchase batteries for the radio today.
Standard #: 22VAC40-185-580-C Description: Based on observation the center did not have all required information in a vehicle used for transportation. Evidence: On 5/9/16, there were no emergency phone numbers in bus (VEV-7166). These numbers include 911 or local dial number for police, fire and emergency medical services and the number of the regional poison control center. Plan of Correction: I will correct this today.
