Discussed with the Director: Remove expired medication permission forms from the binder and file in the child's file. Watch for the form that is for the public school to give medication. That is not the same as your form. Also discussed increasing supplies now that your todddler room has grown.
Unannounced monitoring inspection was conducted beginning at 9:20 and ending at 12:00. There were 38 children present with 9 staff. Children were ages 6 months - 5 years of age. Ratios were appropriate in all classrooms. Children were observed having morning snack of oranges and crackers., listening to stories, doing table top activities. Babies were observed being, fed, played with and cuddled. Classrooms were brightly decorated with children's work. Standard #: 22VAC40-185-60-A Description: Based on a review of 6 children's files, it was determined that 2 files were incomplete. Evidence: The file for child 3 who was enrolled on 5/2/16, did not have physicians's name and phone number and had no emergency contacts. The file for Child 6 , who was enrolled on 3/14/16 did not have a current shot record. Plan of Correction: The Director will assure that files are complete.
