Technical Assistance:
Discussed with provider getting clarification of medication, prescription label should reflect what is written on the medication form. Discussed getting complete addresses for the two emergency contacts.

Comments:
An unannounced monitoring inspection was conducted from 10:15 am-2:00. At the time of entrance, 100 children were in care with 16 staff members present. Children were observed engaged in group activities and music . A selection of staff and children records, medications and other documents were reviewed. The new playground, surfacing and equipment was inspected and measured. The violations were reviewed with the center director. Standard #: 22VAC40-185-70-A
Description: Based on a review of staff records, licensee did not maintain a complete staff record. Evidence: Staff # 1, hired 2/1/2016 does not have documentation of 2 references.

Plan of Correction: This was a former employee two references were obtained by COB on 5/5/16.

Standard #: 22VAC40-185-80-A
Description: Based on a review of attendance records, the center has not maintained written record of children in attendance each day. Evidence: Several classrooms attendance record was not completed for the month of May and one not completed since 4/26/2016.

Plan of Correction: Attendance was updated and staff have been reminded to complete daily. Corrected at time of visit.

Standard #: 22VAC40-185-280-B
Description: Based on classroom observation, licensee did not maintain hazardous substances in a locked place. Evidence: A bottle of bleach was observed in the top cabinet in room # 159. This cabinet was not locked. A bottle of bleach and Antibacterial spray was found in a bottom cabinet in the Kindergarten room. The chemicals were not locked and accessible to the children.

Plan of Correction: Corrected at time of monitoring visit. Bleach and antibacterial spray removed from room to a locked location. Future action- temporary lock installed by 5-9-16. Permanent lock installed 7-2016 to our new facility cabinets.

Standard #: 22VAC40-185-280-G
Description: Based on observation and staff interview, licensee has not clearly marked the containers to identify the chemical content. Evidence: A bottle of liquid was found in the top cabinet in room # 159, staff indicated that the liquid was a water and bleach mixture. The bottle was not labeled to identify its contents.

Plan of Correction: Corrected at time of monitoring visit. Bleach water solution was labeled and a plan was put into place for fututre when bottles are replaced.
