Description: Based on documentation and interviews the facility failed to appropriately document the staff member administering a medication. Evidence: 1. On 05/13/2016 the licensing inspector reviewed documentation and there was an error made in the documentation of the staff member who administered one dose of a medication for child #1. 2. On 05/13/2016 staff #1 and staff #2 confirmed there had been an error made in the documentation of the administration regarding one dose of medication for child #1.

Plan of Correction: We were able to present to the licensing inspector exactly who the member was that administered the one dose of medication for the child. We will review this with all MAT trained staff in order to ensure it doesn't happen again.

A compliance history is in no way a rating for a facility.

The online compliance history includes only information after July 1, 2003. In addition, the online compliance history includes information regarding adverse actions that may be the subject of a pending appeal. An adverse action is not final until a provider has exhausted or waived all due process rights. For compliance history prior to July 1, 2003, or information regarding the status of pending adverse actions, please contact the Licensing Inspector listed in the facility's information. The Virginia Department of Social Services (VDSS) is not responsible for any errors in or omissions from the compliance history information.
