Today's visit was a routine monitoring inspection. The inspection began at 8:30am and concluded at 5:00pm. There were one hundred ten children present with twenty-two direct care staff members. During the inspection all eleven classrooms were observed, lunch time was observed, infant care was observed, diapering was observed, hand washing was observed, nine out of twenty-nine children's records were reviewed, nap time was observed emergency supplies were viewed, all twelve medications along with authorizations were reviewed, afternoon snack time was observed, seven out of one hundred fifty-nine children's records were reviewed, some policies and procedures were reviewed, and a sample of injury reports were reviewed. Standard #: 22VAC40-185-210-A Description: Systemic Deficiency- Based on the inspector's review of nine out of twenty-nine staff and classroom observation, the center failed to ensure a Program Lead for a grouping of children had documented required qualifications. Evidence: The sole staff member (#1) for the Blue Bunnies I three year old classroom does not have documentation of the completion of high school or high school equivalent as required by the standard. Plan of Correction: The PD has requested that the staff member provide documentation of her completion of high school or equivalent.
Standard #: 22VAC40-185-350-C Description: Based on the inspector's observations during a class instructed in the multipurpose area, the center failed to ensure the group was within ratio according to the youngest child. Evidence: The inspector observed a soccer class in the multipurpose area. The group consisted of children two and three years of age. There was one staff member with ten children. Another individual instructing the class did not count in the staff to chid ratio as they are contracted and do not have a staff record with all of the required elements. The ratio applies to the youngest child in the group and is one staff member for every eight children when a two year old child is present. Plan of Correction: The PD will ensure additional staff are present with the contracted staff are conducting a group class or class sizes will be adjusted.
Standard #: 22VAC40-185-500-B Description: Based on the inspector's observations in the Red Bears II classroom, the center failed to ensure staff used a foot operated disposal system when conducting a diaper change. Evidence: The inspector observed a diaper change in the Red Bears II classroom. The staff member used her hands to open the lid of a trash can to dispose of a soiled diaper and additional diapering supplies. Plan of Correction: The PD will review with all staff who are responsible for changing diapers the required diapering procedures.
Standard #: 22VAC40-185-510-G Description: Based on the inspector's review of all twelve medications stored in the center's lock box, the center failed to ensure a prescription medication was labeled with a prescription label. Evidence: The inspector viewed a box with Mupirocin a prescription medication as indicated on the labeling on the box. The box/medication does not have a prescription label as required by the standard. There is doctor's authorization for use. The medication has been logged as administered to the child. Plan of Correction: The medication will be returned at the end of the program day. Prescription medication will not be accepted at the center without an original prescription label.
Standard #: 22VAC40-185-570-A Description: Based on the inspector's observations in the Purple Ponies II classroom with children ages sixteen months through two years of age, the center failed to ensure safety straps were in place or in use when two children were observed seated in high chairs. Evidence: The inspector observed nine children seated in a feeding table and two in high chairs. One child seated in a high chair did not have the safety straps fastened and one child's high chair was missing the safety straps completely. Plan of Correction: The PD will ensure all staff understand to request additional safety straps when straps are broken or missing and the requirement to have safety straps in use when equipment is manufactured with safety straps.
