22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted on May 17th 2016 between 12:00pm and 2:00pm. There were 13 children in the direct care of 3 staff members. Children were observed eating lunch, preparing for nap, napping, and participating in arts and crafts. A sampling of children's records, staffing records, emergency preparedness plan, physical space, supervision, and medication were observed and inspected. Standard #: 22VAC40-185-160-A Description: Based on record review, a staff member did not have documentation of a Tuberculosis screening that was completed 12 months prior to employment. Evidence: Staff #1 was hired on 2/24/2016. The staff member had a Tuberculosis screening on record that was dated 07/11/2014 which is more than 12 months prior to the hire date. Plan of Correction: Will have her update her Tuberculosis test.
Standard #: 22VAC40-185-240-D-1 Description: Based on medication review and staff interview, the center had a staff member not certified to give medication issuing medication. Evidence: On 5/5/2016 a medication for child #1 was administered by a staff member that does not have Medication Administration Training. The staff that is certified stated that she was present and supervised the medication administration. Plan of Correction: The staff member that issued the medication will be MAT certified in 45 days. Until then the MAT certified staff member will administer the medication.
Standard #: 22VAC40-185-510-G Description: Based on medication review, the center did not follow the orders on the prescription label of 1 medication. Evidence: Child #4 medication had an instruction on the prescription label that the medication should be discarded after September 2015. The medication was not discarded and was used 8 separate times between September and April. Plan of Correction: We documented the expiration date of April 2016 assuming that the expiration date was the date to follow. We did not notice that the discard date was different.
Standard #: 22VAC40-185-560-G Description: Based on observation, food brought from home was not clearly dated. Evidence: During lunch time, the lunch boxes on the tables did not have a date on the container. Plan of Correction: We will assign assistant teacher the responsibility of putting labels on the lunch boxes in the morning.
