22VAC40-191 Background Checks (22VAC40-191)

An unannounced monitoring visit was done today May 10, 2016 beginning at 10:00 AM and ending at 2:30 PM. There were twenty- two children in care today. The children were observed during their play time in the gym where they had been playing soccer and then sleep tag". The children had also made a theme related craft and were reviewing their colors and shapes. The teacher also read a story while using a puppet. The children also observed outdoor time. Lunch was observed as well which is packed and brought from home. The children napped in the afternoon. The inspector reviewed a sample of staff and children's records. There was no medication on site today for the preschool .The violations and corrective action will be discussed with the Director at a future date. Description: Based on observation the center did not check to determine all the immunization forms were correctly signed by a physician or his designee. Evidence: 1. During the inspection the LI reviewed the files of five children and child # 3 had an immunization however it was not signed by a physician or his designee.

Plan of Correction: We have requested a new immunization form from physician to include the signature.

Description: Based on observation the center did not obtain all of the required information for the children enrolled. Evidence: 1. During the inspection the LI reviewed the files of five children and child # 1 was in need of an additional emergency contact

Plan of Correction: We have obtained and documented a second emergency contact for this student.

Standard #: 22VAC40-185-550-D
Description: Based on observation the center did not practice the required monthly fire drill. Evidence: 1. Based on documentation provided to the LI , the fire drills were not practiced monthly. The documentation indicates they were practiced in the months of 8/15, 9/15, 1/16 and 4/16.

Plan of Correction: The director will ensure the fire drills are practiced monthly in the future.
