Standard #: 22VAC40-185-270-A
Description: Based on observations, it was determined that one area of the school has not been maintained in a completely clean condition and two areas have not been maintained in a completely safe condition. Evidence: 1. In the Monkeys/Fish classroom, a toilet cleaning brush was stored on the floor directly next to the children's toilet. 2. In the Owls classroom, there is a bookshelf containing dress up and dramatic play materials. This bookshelf does not stand flat on the floor and is not secured to the wall, making it top heavy and a possible hazard because a child could pull it over. 3. On the playground, the plastic storage box containing some of the children's outdoor toys is cracked and split.

Plan of Correction: 1. The toilet brush was left there by the cleaning company. It will be moved immediately. 2. The bookshelf will either be secured to the wall or replaced before the start of school in the Fall. 3. I have already ordered a new storage box that will be installed before the start of school in the Fall,

Standard #: 22VAC40-185-280-B
Description: Based on observations, it was determined that potentially hazardous substances were unlocked in various classrooms. Evidence: 1. Unlocked upper level cabinets containing cleaning products were observed in the following classrooms: Monkeys, Fish, Pandas, Science Room, Butterflies. 2. In the Monkeys classroom, a container of floor cleaner was stored connected to the mop in the children's bathroom. 3. In the Fish classroom, a bottle of hair spray was stored in an upper level unlocked cabinet.

Plan of Correction: 1. The cabinets with unlocked cleaners are used only by the church for Sunday school. Staff will be required to check those cabinets prior to opening their classroom each morning to make sure they're locked. 2. The floor cleaner will be removed from the mop. 3. The hair spray will be removed or locked.

Standard #: 22VAC40-185-280-G
Description: Based on observations, it was determined that two spray bottles of cleaning products were not labeled as required. Evidence: In the Butterflies classroom, two spray bottles of table cleaning solution were not labeled to indicate the contents.

Plan of Correction: The bottles will be labeled immediately.

Standard #: 22VAC40-185-330-B
Description: Based on observations, it was determined that one area of the playground does not have enough resilient surfacing. Evidence: There is not enough mulch beneath the half circle climber. In this area, the mulch has worn down to dirt.

Plan of Correction: The mulch will be redistributed and additional mulch will be installed before the start of the new school year.

Standard #: 22VAC40-185-510-J
Description: Based on observations, one adult medication was not locked. Evidence: In the Caterpillars classroom, a container of adult non-prescription medication was stored in an upper level unlocked cabinet.

Plan of Correction: The medication was moved to a locking cabinet immediately during the inspection.
