1. Discussed loose turf on the pre-school age playground as management will need to monitor to ensure it does not become a trip hazard. 2. Recommended that the off site evacuation location is closer to the center. The existing evacuation site is located in Burke.

Comments:
Conducted an announced initial inspection from 10:30am until 1:15pm. Interior and exterior of center inspected, classrooms measures and records reviewed. No children present as the planned opening date is 06/06/16. The center was found to be very clean and exceptionally well supplied. Classrooms measured. The facility will offer infant playground, preschool age playground, school age playground, sport court and water play area. The school bus was in compliance. Policies, procedures and emergency preparedness plan were in order. The center has demonstrated readiness for licensure and it is recommended a 6 month conditional license be approved.

Violations:
Standard #: 22VAC40-185-250-A
Description: Based on records reviewed, it was determined the center did not have the final Fire Permit issued by the local jurisdiction.

Plan of Correction: obtaining Fire Permit immediately (assembly permit)

Description: Based on observations made it was determined that over the counter medications were not locked. Evidence: Approximately 6 over the counter pain reliever products were stored in a unlocked First Aid Kit mounted on the wall in a hallway adjacent to classrooms.

Plan of Correction: immediately removed all medications from First Aid Kit and will adjust maintenance of First Aid Kit deliveries.
