Technical Assistance:
All dates for short term medications should be accounted for on center logs to ensure administration is complete. Pick up time listed on injury forms must be a specific time rather than the phrase "at pick-up"

Comments:
An unannounced renewal study was conducted today. All rooms were in ratio with direct care staff. Observed diapering and infant care, transitions to circle time, centers and free play, writing letters in pre-k and lunch time time and transition to rest time. Classrooms are well equipped and reflect children's work. Records for staff and children, emergency preparation and care of children as well as required annual trainings, MAT, First Aid and CPR are all up to date. Areas of non-compliance are identified in this report. Please sign and date the report and return it to me within 10 calendar days from date of receipt. Standard #: 22VAC40-185-60-A
Description: Based on record review, an allergy action plan would not be able to be implemented as one medication was not on-site. Evidence: An allergy action plan for child #1 had an antihistamine to be administered if there was a mild allergic reaction but the medication was not on-site.

Plan of Correction: Parents notified. Must bring Benadryl or face suspension til available.

Standard #: 22VAC40-185-70-A
Description: Based on record review, 2 of 2 staff references were not complete. Evidence: The references for staff #1 and #2 did not contain the signature of the staff member conducting the reference.

Plan of Correction: Documents signed.

Standard #: 22VAC40-185-260-A
Description: Based on record review, the annual fire inspection permit is not posted. Evidence: The posted fire inspection report expired 4/30/2016.

Plan of Correction: Fire Marshal contacted. Scheduled Inspection 5/31/2016.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the diaper pad in one classroom is no longer non-absorbent. Evidence: The diaper pad in Infant II classroom had a small tear in the plastic lining and would no longer be considered non absorbent.

Description: Based on observation, a medication in a classroom was not properly labeled. Evidence: The medication for a child in the classroom emergency backpack was not labeled with identifying information (name)

Plan of Correction: The classroom teacher labeled the medication with the child's name during the inspection.

Standard #: 22VAC40-185-540-E
Description: Based on staff management statement, a non medical emergency supply is not available. Evidence: No weather band radio was available.

Plan of Correction: Radio-solar crank purchased in Director's office.

Standard #: 22VAC40-185-550-A
Description: Based on center management statement, the emergency preparedness plan needs to be updated. Evidence: The emergency plan's secondary officer needs to be updated as the listed staff member is no longer employed at the center.

Plan of Correction: Updated and posted.

Standard #: 22VAC40-185-550-L
Description: Based on a sample of the injury forms for minor injuries, the forms are not completed with all required information. Evidence: 5 of 15 injury forms did not include a time the parent was notified.

Plan of Correction: Updated reporting procedure-all staff review.
