22VAC40-191 Background Checks (22VAC40-191)

Technical Assistance:
Two components are required for snack. Salsa does not count as a component for snack. Shaded areas are required for June, July, and August. Summer packets should include sunscreen authorization, insect repellant authorization, and permission for swimming and child's swimming ability.

An unannounced renewal study was conducted with center owner, center director, and center staff. There were 66 children present, ranging in age from three months to ten years old. The children were having rest time, eating afternoon snack, and arriving from school. Children went outside to play, worked on homework, and made a variety of art projects. Two children's files and three staff files were reviewed - please see Supplemental Summary for missing items. The last health inspection was conducted 12/21/2015. Snack served today: chips, salsa, water, and milk. The rooms were measured per request of the owner. Standard #: 22VAC40-185-140-A
Description: Based on review of two children's records, the facility failed to have documentation of a physical on file for each child. Evidence: Child A (date of enrollment 8/15/2015) did not have documentation of a physical on file.

Plan of Correction: Facility will request this information from the parents.

Standard #: 22VAC40-185-160-A
Description: Based on review of three staff files, the facility failed to have documentation of a tuberculosis test or screening within 21 days of hire. Evidence: Staff A( date of hire 3/29/2016), Staff B (date of hire 2/1/2016) and Staff C (date of hire 3/7/2016) did not have documentation of a completed TB test or screening.

Plan of Correction: Staff A, B, and C will get documentation of a TB test or screening.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the facility failed to maintain the areas outside the center in a clean and safe condition. Evidence: there were several large mushrooms growing in the mulch on the large playground.

Plan of Correction: Staff will remove the mushrooms from the playground.

Standard #: 22VAC40-191-40-D-1-B
Description: Based on review of three staff records, the facility failed to have a completed search of the central registry Child Protective Services check before 30 days of employment at the facility ends. Evidence: Of three files reviewed, Staff C (date of hire 3/7/2016) did not have documentation that the search of the central registry was completed.

Plan of Correction: Staff called VDSS OBI to check on the status of the CPS check for Staff C.
