Comments:
An unannounced monitoring inspection was conducted with center director and center staff. There were 31 school age children present with three staff supervising. The children were released from school early today, and were in the cafeteria making paper airplanes. Outdoors the children jumped rope, played basketball, blew bubbles, and flew the paper airplanes. Five emergency medications for five children were reviewed - please see Supplemental Summary for missing item. Standard #: 22VAC40-185-510-A
Description: Based on review of four medications, the facility failed to get written parental authorization for each medication on site. Evidence: the parent had not signed the long term medication authorization for Child A's epipen. The physician had signed the form.

Plan of Correction: Child A is not scheduled to return to care this school year; medication will be returned to parent.
