Comments:
An unannounced monitoring inspection was conducted with center staff. There were 11 school age children present with two staff actively supervising. The children were having free play with age appropriate activities and playing a group gym game. Two staff files, one child's file, and one emergency medication were reviewed. Please see the Supplemental Summary for missing items. Be sure staff files are complete, including staff who float from one center to another. Violations:
Standard #: 22VAC40-185-160-C
Description: Based on review of two staff records, the facility failed to have documentation of a current TB test or screening for each staff every two years. Evidence: the tuberculosis test for Staff A expired 10/15/2015.

Plan of Correction: Staff A will obtain documentation of an updated TB test or screening.

Standard #: 22VAC40-185-510-C
Description: Based on review of medication, the facility did not prevent the use of outdated medication. Evidence: Child A's epipen expired 1/2016, but had not been returned to the parent or disposed of by the center.

Plan of Correction: All medications will be returned to the parents tomorrow.

Standard #: 22VAC40-185-510-N
Description: Based on review of medication, the facility failed to send the medication home within 14 days of the expiration of the medication authorization. Evidence: the long term medication authorization for Child A's epipen expired 5/4/2016. The medication had not been picked up by the parent within 14 days or disposed of by the center.

Plan of Correction: Medication will be returned to parent tomorrow.

Standard #: 22VAC40-191-60-C-1
Description: Based on review of two staff records, the facility failed to have documentation of a criminal history record check for each staff. Evidence: Staff A (date of hire 8/13/2013) did not have documentation of a criminal history record in the file on site.

Plan of Correction: Staff A will check with the main office to obtain a copy of the criminal history record. If it can not be located, Staff A will not be able to return to work until it is completed again.

Standard #: 22VAC40-191-60-C-2
Description: Based on review of two staff records, the facility failed to have documentation of a search of the central registry in each staff's file. Evidence: Staff A (date of hire 8/13/2013) did not have documentation of search of the central registry check in the file on site.

Plan of Correction: Staff A will check with the main office to obtain a copy of the search of the central registry check for the site file. If it can not be located, Staff A can not return to work until it is completed again.
