22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted today from 10:30 am-12:00 pm. There were 40 children directly supervised by 9 staff. The physical plant, staff records, children' records, children's medications records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed playing with the new bikes outside, practicing putting on jackets while lining up to go outside and teachers engaged with babies at their level encouraging walking and tummy time. There was an abundant supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, staff-to-child ratios were in compliance. The center was clean and organized. Standard #: 22VAC40-185-60-A Description: Based upon record review, children's records were found to be incomplete. Evidence: Child #1's record is missing emergency contact #2. Plan of Correction: Rec'd 2nd emergency contact. Will ensure moving forward to collect upon enrollment.
Standard #: 22VAC40-185-270-A Description: Based upon observation, areas and equipment are in need of repair. Evidence: Pre-school classroom - red sofa and chair ripped and strong smell of urine coming from drain in boys bathroom. 2's classroom- broken play food and cabinet by classroom door has areas of chipped wood. Older playground- tripping hazard in grass area ( small black sqare in the middle of the grass. Mulch on playground was observed to be much lower than the sidewalk creating a tripping hazard along the inside edge the entire side walk. The picnic table under the canopy has splintered wood and needs to be sanded. Multiple sections of the playground fence were observed with pretruding nails and/or leaning and not sturdy. younger playground- Broken car needs to be removed, rusted paint on metal wagon ride on toy. Plan of Correction: Will remove/ replace all broken items and coach teachers moving forward on their responsibility around disposal of broken items. Work orders were placed surrounding facility/ playground issues, again.
Standard #: 22VAC40-185-280-B Description: Based upon observation, chemicals were not kept in a locked place that prevents access by children. Evidence: Bleach water cleaning solution was observed in an unlocked cabinet in the pre-k classroom above the children's hand washing sink. Plan of Correction: Management will remind all staff and will monitor more closely moving forward.
Standard #: 22VAC40-185-430-B Description: Based upon observation, materials in the infant classroom were not appropriate in the current presentation. Evidence: Hand made sensory bottles filled with small erasers, objects from nature and rocks were available to babies. The lids to the bottles were not super glued to prevent access. Plan of Correction: Will not glue lids.
Standard #: 22VAC40-185-500-A Description: Based upon observation, Staff did not wash their hands with soap and running water before/ after any contact with body fluids. Evidence: Teachers in the younger and older infant rooms wiped a child's nose and did not wash their hands after doing so. Plan of Correction: Training on hand washing. Observations moving forward to confirm compliance.
