Comments:
An unannounced inspection was conducted from 2:15pm through 5:00pm. There were 27 children in care ages 17 months through five years old with six staff. Children and staff records were reviewed. Children were observed in teacher and self-directed activities including nap, snack, and restroom routines. Violations were observed in the following areas and appear on the violation notice; labeling diaper cream, unlocked chemicals, ratio, posted emergency numbers.

Violations:
Standard #: 22VAC40-185-280-B
Description: Based on observation and interview the center failed to ensure that hazardous substances are kept in locked place using a safe locking method that prevents access by children. Evidence: 1 - The kitchen was unlocked during the inspection. Inside the kitchen are a plastic container of bleach and a bottle of hand sanitizer. These items are within reach of children. 2 - Staff 1 confirmed that the kitchen was unlocked and that there are accessible chemicals inside.

Plan of Correction: Center management will remind and retrain all staff in the need for the kitchen to be kept locked when there is not anyone in the kitchen.

Standard #: 22VAC40-185-350-E-4
Description: Based on observation and interview the center failed to ensure that for children from three years of age to five years of age: one staff member for every 10 children. Evidence: 1 - At 2:30pm until 2:51pm there were 11 children up and moving around the classroom with one staff present. The number of children present requires two staff members. 2 - Staff 2 and staff 1 confirm that there were 11 children present in the classroom with one staff.

Plan of Correction: Center management will ensure that the correct staff to child ratio are maintained during all hours of operations.

Standard #: 22VAC40-185-520-C
Description: Based on observation and interview the center failed to ensure that diaper ointment is labeled with the child's name. Evidence: 1 - There is a tube of diaper ointment in the bathroom that is not labeled with the child's name. 2 - Staff 1 confirmed that the diaper ointment was not labeled with the child's name.

Plan of Correction: Center management will remind and retrain all staff in the necessity for each child's diaper ointment to be labeled with the child's name.

Standard #: 22VAC40-185-550-F
Description: Based on observation and interview the center failed to ensure that emergency numbers are posted in visible place at each telephone. Evidence: 1 - The telephone in the hallway between the classrooms does not have emergency numbers visibly posted neat it. 2 - Staff 1 confirmed that the required emergency numbers were not posted near the telephone.

Plan of Correction: Center management will ensure that the required emergency numbers are posted near each phone.
