An unannounced monitoring inspection was conducted from 12:40 p.m.-3:00 p.m. with the center director and assistant director. 39 children ranging in age from 5 months through 5 years were in the care of six staff. Children were observed having nap time, reading a book, and eating afternoon snack. Afternoon snack served today was baked apple cinnamon bars and milk. The last emergency escape drill was 6/3/16. Both the fire and health inspections are current. Three children's records were reviewed. Three staff records were reviewed and in compliance. Standard #: 22VAC40-185-140-A Description: Based on review of three children's records, the center did not have documentation of a physical in each child's file prior to or within 30 days of first attendance. Evidence: Child A's (enrolled 3/14/16) record did not have documentation of a physical. Plan of Correction: We will notify the parent at pick-up today.
Standard #: 22VAC40-185-280-B Description: Based on observation, the center did not keep hazardous substances such as cleaning materials in a locked place to prevent access by children. Evidence: The unlocked utility room in the early early preschool classroom had a container of bleach and two buckets of paint. Plan of Correction: The director immediately locked the utility room door. We will speak with the teachers that access that room.
Standard #: 22VAC40-185-330-B Description: Based on inspection of the playground, it was determined that the mulch under and around play equipment was not resilient. Evidence: The depth of the wood chips was measured in three spots around the center composite play structure on the preschool/school age playground. All three spots measured about one to two inches in depth. The required resilient surface depth is at least nine inches due to the height of the structure being about 7.5 feet. The depth of the wood chips was measured in three spots around the side Little Tikes composite play structure on the preschool/school age playground. All three spots measured about one to one and half inches in depth. The required resilient surface depth is at least six inches due to the height of the structure being about 6 feet. Plan of Correction: Both playground structures must remain off-limits to children until the minimum depth of wood chips is replenished. A work order was already placed. A second work order, which will include the licensing inspection report, will be sent in today. We will contact our district manager too.
Standard #: 22VAC40-185-430-C Description: Based on inspection of the playground, the center did not maintain play equipment as required. Evidence: Both s-hooks of the left swing on the large preschool/school age playground were open more than the thickness of a penny. Plan of Correction: The left swing may not be used by the children until the s-hooks have been tightened. A work order will be put in today.
