Comments:
Arrived 9:00am Departed 11:30am An unannounced monitoring inspection was conducted today. Ninety-four children were present with 23 staff. The children were playing centers, having water day, reading stories, playing outdoors and eating lunch. Two staff files and six children's files were reviewed for compliance. Thank you for your cooperation. Please contact me if I may be of service.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation the facility did not insure that all areas of outdoor play are in a safe condition. Evidence: 1. On the gate between the preschool and schoolers' playgrounds, there is a bolt protruding three inches from the wooden post. It is approximately four feet off the ground and presents a protrusion hazard.

Plan of Correction: The owner reported that the maintenance person will repair or remove the bolt. Until that can be scheduled center staff will cover the bolt with foam to eliminate the protrusion.
