The licensing inspector conducted an unannounced monitoring inspection June 14, 2016 from approximately 9:50am to 1:10pm. The following ratios were observed with a total of 134 children present during the inspection:
Satellite Infants I - 2 children with 1 staff
Satellite Infants II- 2 children with 1staff
Young Infants - 4 children with 2 staff
Older Infants -8 children with 1 staff
Toddlers (Red Room) - 13 children with 3 staff
Twos (Orange Room) - 16 children with 3 staff
Twos (Yellow Room) - 16 children with 3 staff
Threes (Green Room) - 19 children with 2 staff
Threes (Blue Room) - 19 children with 2 staff
Fours & Fives (Purple Room) - 18 children with 2 staff
Fours & Fives (Gold Room) - 15 children with 2 staff
Get Well - 2 children with 2 staff
During the inspection, the Infants were observed playing on the floor with toys, eating, sleeping in their cribs, and being held and fed. The Toddlers were eating lunch and playing outside. The Twos classrooms were observed building with legos, looking at books, building with magnet blocks, playing in station square and creating Father?s Day gifts. The Threes classrooms were observed playing outside, working puzzles, writing on wipe off boards, washing their hands and eating lunch. The Fours and Fives were observed collecting items to build a home for fireflies, playing outside, washing their hands, playing in dramatic play center, looking at books and eating lunch. Staff was observed interacting with the children and providing assistance when needed. Lunch consisted of ham and cheese roll-ups, cucumbers, fruit salad and milk. All areas of the center including classrooms, hallways, bathrooms, kitchen and playgrounds were inspected. The center was equipped with toys and supplies and items were available to the children.
The following information was reviewed: emergency supplies, liability insurance, injury reports, fire and shelter-in-place drills, daily attendance, menu, parent agreements, and staff training information. Medication is administered and medication and medication authorizations were inspected.

During the inspection, five children's records were reviewed. Because the director was on vacation the staff records will be reviewed during the next inspection. All required information regarding health concerns and emergency contacts were available during the inspection. Standard #: 22VAC40-185-290-3 Description: Based on observation, the center did not ensure all electrical outlets had a protective cover. Evidence: During the inspection, the licensing inspector observed an electrical strip sitting on the floor beside a refrigerator in the Satellite Nursery. Two of the electrical outlets did not contain protective covers. Plan of Correction: Corrected during the inspection.
Standard #: 22VAC40-185-440-B Description: Based on observation, the facility did not ensure all cribs were identified for use by a specific child. Evidence: During the inspection, the licensing inspector observed the Satellite nursery did not have an established system for identifying one of the cribs for a child that was using the crib. Other cribs located in the room contained a name tag for the Infants that used the cribs. Plan of Correction: Corrected during the inspection.
Standard #: 63.2-1720-F Description: Based on observation, the facility permitted one staff member to work alone with children before a satisfactory criminal history record was obtained. Evidence: During the inspection, the licensing inspector observed the following: 1. Staff #1 (hire date 05/17/2016) was observed supervising nine children in the Purple Room (Fours & Fives). Staff #1 was observed alone in the classroom with the children from approximately 11:25am-11:40am. Nine other children from the Purple Room were observed playing outside with another staff member. Members of management requested a copy of the criminal history record for Staff #1 from the corporate office while the licensing inspector was on site. Staff from the corporate office stated a completed criminal history record had not been received. The criminal record check had been sent to the proper agency for processing the first week of employment for Staff #1. Staff are not permitted to work in a position that involves direct contact with a child receiving services until an original criminal history record has been received, unless the staff remains within sight and sound / direct supervision of another staff for whom a background check has been completed. Plan of Correction: Staffing was readjusted during the inspection and the staff member will not work unsupervised until the criminal history record has been received.
