Comments:
An unannounced monitoring inspection was conducted from 9:10am-11:00am. Upon arrival there were 40 children in care with 6 staff present. A sample of 2 staff and 3 children's records were reviewed in addition to 3 medications. Children were observed finishing morning snack, during toileting and on the book mobile.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation the center failed to ensure all areas and equipment of the center, inside and outside were maintained in a safe condition. Evidence: Two swings on the playground had rust on the metal areas of the seat and chains.

Plan of Correction: The director will have maintenance paint the rusted areas on the swing seats and replace the metal chains.

Standard #: 22VAC40-185-330-B
Description: Based on observation the center failed to ensure where playground equipment is provided resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99. Evidence: The merry-go-round, Little Tykes Slide set and swings had 1inch of resilient surfacing in the fall zones. Six inches is required.

Plan of Correction: The director will order additional mulch for the playground. The children will play in the grassy area of the playground until the required amount of mulch is delivered.

Standard #: 22VAC40-185-340-F
Description: Based on observation the center failed to ensure children under 10 years of age always shall be within actual sight and sound supervision of staff. Evidence: When the inspector arrived at 9:10am there was a 4 year-old child in the hallway next to the director's office tying his shoe. There was no staff present except for the a volunteer music teacher. The child's teacher and class were in the director's office, leaving the child without sight supervision of a qualified staff person. Several minutes went by before the child joined his class in the director's office.

Plan of Correction: The director will address this with all staff today. Supervision policies will be discussed with all staff. The director will monitor this for future compliance.
