Comments:
An unannounced monitoring inspection was conducted today, June 16, 2016 to determine the center's compliance with licensing standards. The inspector arrived at 12:30 pm and concluded the inspection at 3:30 pm. Upon the inspector's arrival, children were napping. Later children were observed engaged in various activities. The preschool children worked had a story before going outside. Upon returning to the room, children worked on cards for their father's day gift to Dad. The two year old classroom was observed having snack and hand washing, Children had time outside on the play ground. The toddler class celebrated a birthday of a classmate with cupcakes. Later children played with push toys, blocks and the kitchen set. Infants had time on the floor with soft toys and snack in the highchairs. Diapering was observed. Staff held and talked with children in a positive manner. Today was the last day of public school. School agers arrived at noon and watched a movie before having a end of school year dance party. Cupcakes and special snacks were served. Friday the facility will be hosting "Donuts with Dad" for Father's Day. Each class has made a gift and card for the Dads. The playground area was inspected. The classrooms are decorated with the children's artwork and are equipped with age and stage appropriate materials. New carpet has been installed throughout the center. The center had the following posted: license, daily schedule, emergency numbers, evacuation maps, menu and various parent information. The center's first aid kit and emergency supplies were inspected and found complete for the building and the Bus. Summer camp starts Monday for the school age children. Four employee records and five children's records were reviewed during today's inspection. Emergency medications and authorizations were reviewed. Two staff have current EMAT certification. Health Inspection: 08/12/2015 Fire Inspection: 10/08/2015 The last documented evacuation drill was held on 05/31/2016. The two required shelter-in-place drills for 2015 and 2016 were conducted on on 03/12/2015, 12/15/2015, 02/25/2016 and 03/22/2016. Today the following ratios were observed: Pre-K: ( 4 and 5 year old) 17 children with 2 staff. Toddlers: 12 toddlers with 3 staff. Preschool: (2 and 3 year old) 14 children with 2 staff. Infants: 8 infants with 2 staff. School age children: 35 children with 2 staff. The monthly menu was posted and today's lunch consisted of: bologna sandwich, corn, pears and milk. The violations from the previous inspection on November 20, 2015 were checked for corrections. There were no repeat violations found during today's inspection. Standard #: 22VAC40-185-270-A
Description: Based on observations, the facility did not ensure areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: 1. Eight seat on the school bus, used to transport children during the summer on field trips, were torn and had exposed foam posing a choking hazard. 2. A wooden timber used as a barrier for the resilient surfacing around the climbing equipment on the playground had deteriorated and was pulled up from the ground on one end exposing a jagged wooden edge and posing a tripping hazard.

Plan of Correction: Per the Owner: 1. I will repair the bus seats to make sure the foam is not exposed. 2. I will remove the timber and have it replace with a new one.

Standard #: 22VAC40-185-330-B
Description: Based on observation and measurements taken, the facility did not ensure where playground equipment is provided, that the resilient surfacing complied with the minimum safety standards. Evidence: The following six measurements were taken around the climbing equipment on the preschool/school age playground: Two inches at the bottom of the slide shoot, five inches next to the swing bridge, two inches to the right of the slide, less then one inch at the bottom of the shorter slide shoot (dirt was visible), three inches to the left of the shorter slide and five inches at the rear of the equipment. Six inches is required for the depth of the pea gravel resilient surfacing. The resilient surfacing did not meet the requirement. Additional pea gravel was observed in areas where equipment is not located.

Plan of Correction: Per the Owner: I will have someone move the additional pea gravel from the open areas of the playground where no equipment is located to the fall zone of the climbing equipment to meet the six inches of resilient surfacing. We will replace the timber that is broken to help maintain the pea gravel that is in place. If additional gravel is needed we will have it installed.
