Comments:
The licensing inspector conducted an unannounced monitoring inspection Wednesday, May 4, 2016 from 10:15am to 11:25pm. The center was recently licensed and is on a conditional license. This was a 60-day monitoring inspection. The children were observed engaged in reviewing the letter of the week in large group time and sensory play with shaving cream and dry oatmeal. The infants were observed being fed and playing on the floor. Interviews were held with staff throughout the inspection. Additionally, when appropriate the inspector interacted with children in each classroom. All classroom areas and the playground were inspected today as well as the facility?s van. Three children?s records and two employee records were reviewed during this inspection. Total enrollment today: 26 Today, the following child to staff ratios were observed: Infants ? 6:2 Toddlers ?4:1 Twos ? 8:1 Preschool?8:1 School Age ? not present The violations from the previous inspection were checked for corrections. Standard #: 22VAC40-185-90--A
Description: Based on a review of children's records on 05/06/2016, the center did not ensure that two of the four records reviewed contained a written agreement between the parent and the center. EVIDENCE: 1. The written agreements in the records of Child #1 were between the parent and the previous owner of the center. The agreement in the record of Child # 1 was signed on 06/22/2015. This date is prior to the start date of the current license, 02/19/2016. 2. The record of Child #2 did not contain documentation of the signed written agreements. A written agreement between the parent and the center shall be in each child's record by the first day of the child's attendance. The agreement shall be signed by the parent

Plan of Correction: Will have forms updated with the new owner's information by 5/5/2016.
