22VAC40-185 SPECIAL SERVICES.
Today's visit was a follow-up inspection due to the issuance of an Intensive Plan of Correction. The inspection took place from 8:30am through 10:30am. During the inspection all violations that were addressed on the Intensive Plan of Correction were reviewed. The children's records cited at the pervious inspection and indicated on the the I.P.O.C. were not verified as corrected as the children are no longer in care. The center would like a ten day review of the issued Violation Notice. The Violation Notice is due in the licensing office no later than the business day 6/30/16. Standard #: 22VAC40-185-340-D Description: Based on the inspector's observations and review of the record for staff member #1, the center failed to substantiate Program Lead qualifications for a staff member for the Tiny Tigers II classroom. Evidence: The inspector observed the Tiny Tigers II infant classroom and neither of the staff members observed in the classroom have documented Program Lead qualifications. Staff member #1 does not have evidence of the completion of high school or high school equivalent and the PD stated staff member #2 has not completed the required training hours. Plan of Correction: The PD stated she will get documentation for staff member #1's completion of high school or high school equivalent.
Standard #: 22VAC40-185-570-E Description: Based on the inspector's observations in the Tiny Tigers I infant classroom, the center failed to ensure a total of six bottles with formula were dated. Evidence: The inspector viewed the the infant room's refrigerator and observed a total of seven bottles filled with formula and six of the bottles were not dated with today's date. One bottle was dated 6/13 and today is 6/20. Plan of Correction: The PD will delegate one person to receive all infant food and bottles as they arrive in the morning to ensure all items are properly dated before being stored.
